Step 1 : CUSTOMIZED MEASUREMENTS

For your ASA eye surgery, Dr. Sujal & Dr. Manisha will begin by using digital Wavefront technology to thoroughly scan and map the surface of your eye.


Step 2 : PREPARING THE EYE FOR SURGERY

In an ASA procedure, the step of creating a corneal flap is omitted. Instead, just the outermost layer of cells on the surface of the eye are carefully removed, followed by reshaping of the cornea to correct your vision. Unlike other centres, at Samyak Drishti, we use a proprietary treatment profile to remove the cells with the MEL80 excimer laser (TransPRK). This method of removing the cells with the excimer laser has unique wound healing advantages for superior long-term results. The cells which are removed will grow back as part of your healing process. Contact lenses will be used to protect your eyes while they are healing.


Step 3 : WAVEFRONT-GUIDED VISION CORRECTION

Using the Mel 80 excimer laser with a sophisticated software called Topoline with Precise Pulse, Dr. Sujal & Dr. Manisha performs your laser treatment. Their expertise with this advanced laser system allows them to perform your surgery rapidly. During ASA recovery, you can expect a slightly longer healing time than with LASIK. You should have good vision as a result of your surgery in four to six weeks.

FREQUENTLY ASKED Advanced Surface Ablation (ASA) QUESTIONS

In LASIK we make a flap on the outer skin of the eye called the cornea. We then fold it back and apply the laser shots which help in reshaping the cornea and this contributes towards spectacle free vision. We then replace the flap, which heals back in a few hours, In ASA we do not make the flap but apply the laser pulses directly to remove the outer layer of the cornea, followed by application of more laser pulses to correct your spectacle number. In this procedure as there is no flap the outer layer gradually heals back in 2 to 3 days, and eventually you get back the same results as LASIK but it only takes a little longer.

There are some candidates who may not be suitable for LASIK or SMILE. If they still wish to see well without glasses then ASA may be a good option. These patients are whose corneal thickness is too thin or they may have a high cylinder along with a thin cornea or irregular cornea and SMILE or LASIK may not be ideal.If the shape of the cornea is irregular suggestive of early keratoconus, such patients would benefit from topography guided surface ablation. This can also be combined with corneal collagen crosslinking to strengthen and hence improve the stability of the cornea.

Surface ablation takes about 5 to 7 minutes per eye and most patients are surprised at how soon it gets done and feel that it was the quickest part of the journey towards spectacle free vision.

Surface ablation is painless as the surgery is done under the effect of anaesthetic eye drops that numb your eyes to improve your comfort during surgery. After the procedure, until the surface heals, your eyes would feel irritated and you may experience pain and watering along with it for 3-4 days. We will give you some drops and medications to make you more comfortable during the recovery phase.

Few patients may want to do one eye at a time, because while one eye is recovering they can rely on the non-operated eye for their day to day activities, as the recovery is a little longer than LASIK. However, most patients prefer to get both eyes on the same day as it reduces the overall downtime, both eyes recover together and the same medication can be used in both eyes together.

Most people get great vision after surface ablation, though the recovery may be slow. But, a few patients may not be satisfied with the vision quality and may need an enhancement procedure where we do a little bit more of the surface ablation to adjust the results and improve the vision quality. This can be done only after complete recovery and stabilization of the vision which may take upto 6 months.

The recovery from surface ablation is a little prolonged compared to LASIK. Patients often complain of pain, watering and discomfort during the initial phase of recovery, more so in the first 2-3 days. It takes a little longer to recover as a larger surface has to heal compared to LASIK and the vision would fluctuate for a couple of weeks. Typically, by 2 weeks the vision recovers and one can resume all activities.
People with demanding vision needs like photographers, policemen, painters, diamond merchants or others may need to rest a little longer than the others.

Right after PRK most patients see quite well immediately. It’s like a miracle. But, it may not be sharp, and may take a couple of weeks for your vision to get clear. You may be able to see well but it may be a little dull or misty or foggy occasionally. Few patients may feel that they need to increase the fonts on their mobile or computers to see better.
Most people can drive during the day, but could be a little strained at night. We recommend that you take a day at a time during the recovery phase and start driving normally once you are comfortable.

Yes, you can wear colored contact-lenses after surface ablation once the healing is complete.

There is no procedure that is complication free, but when done with due diligence we can expect good results. Surface ablation, in its original form called as PRK, is one of the oldest laser refractive procedures being done since the 1990’s. With a long follow-up available, the chance of unknown side effects or complications of this procedure is unlikely.
In the early days, there wasa higher risk of corneal haze or regression (reversal of the number), but, in the last decade with the introduction of mitomycin-c application following the procedure, the risk is significantly reduced.
